For incoming Kindergarten Students (must be 5 before December 1, 2010) Students applying for the Young Fives Program must have a birthdate between September 1st & December 1st for immediate consideration.

Please bring with you an original or certified copy of the birth certificate we will make a copy and return it to you.

Any Immunizations your child has received to date.

Also bring with you 3 proofs of residency:

1 would be lease, purchase agreement, rent receipt, deed or mortgage statement

2 would be utility bills (gas, electric, waterbill, land phone line, or tax statement)

Copy of Driver's License of Parent/Guardian
